Individual questions may be attached to Child Care Sites along with other locations in the system (Courses and ECFE Sections, Facilities) Questions may also be created as a Question Template then the template can be assigned to those same locations. One advantage of using a Question Template is that any changes that may need to be made only have to be completed one time. Any location that template was used will automatically update to reflect those changes (i.e. to fix a typo, update any details in the questions, etc). Another would be for any situation where staff would need to be able to print the responses (i.e. that someone initialed they have read the refund or media release policy, for confirming acknowledgement of masks or other COVID related policies, etc).
When creating the questions in a template, it can be helpful to use the Information No Answer question type to help prepare the person completing the questions. For example, you could add a note that the next series of questions are related to medical information (allergies, etc) and to be prepared with doctor's contact information. Or, you could explain that the questions may take 10-15 minutes to complete.
If you allow staff to view the responses to questions using the Child Care Attendance app, you may want to ensure the specific questions staff needs are listed first/at the top of the template (so they don't have to scroll as much to see the details they need on the app).
If the list of available templates is getting long, it is recommended to filter the list of templates (i.e. by Created At date) rather than deleting them. There is not a way to reactivate or access a deleted template.

Creating-Enrollment-Questions-including-Conditional-Content-Questions
To build Question Templates, follow the steps below:
- Point to System.
- Select Question Templates.
- Select Add Template.
- Complete the New Question Template form. The text entered in the Name field will be reflected on the Administrative side of the system (not the parent/Public side). Use the Add Question button to add the desired questions. View more details on available Question Types here.
- Select Create.
Note: there is not an automatic save for templates. Staff are encouraged to save often while creating their templates.
